You are invited to nominate a youth volunteer who has contributed to the community over the past year. We would like to thank them for their hard work, caring and dedication. Award recipients will be recognized at the Award Ceremony on Friday, September 16th, 2011 at the Terry James Golf Tournament Dinner.

Criteria

• Must be nominated by a Youth Group Leader, i.e. teacher, coach, registered charity, community organization.
• Individual youth must be 6-18 years of age.
• Nominee has volunteered consistently throughout the past year and has worked on behalf of others, the community or the environment (examples: peer tutoring or counseling, volunteering in community programs/school, raising awareness about an issue, coordination an event, etc.).
• Nominees are a positive role model for their peers.

Award Amount

Each Award will be $500.00

Award Categories

There are six Volunteer Award categories. They are:

• Sport Promotion Award
• Performing/Visual Arts Award
• Community/Social Service Award
• Education Award
• Environment Awareness Award
• Health Awareness Award

How to Apply

Application Deadline is July 31, 2011

Please complete the application form accompanying this brochure OR provide the following information by letter or email:

• Nominee’s Name
• Nominee’s Address
• Nominee’s Phone Number
• Detailed overview of the volunteer project or activity the nominee was involved with including project location, duration, number of people involved, and other specifics about the project
• Describe in detail the impact and benefits of the project or activity on the target audience (i.e. school, community, etc.) as well as the impact the nominee has had on the project
• Provide the names and contact information of three references who can speak knowledgeably about the nominee and the project
• Include supporting documentation where possible (i.e., media clippings, photos, reports, publications, videos). Limit of 5 pieces
• Name of Nominating Person
• Contact numbers of Nominating Person (day/evening phone number)
• Relationship to Nominee (youth group leader, coach, teacher)
